Job Title: Clinical Care Leader (CCL)

Job Summary:
We are seeking a dedicated Clinical Care Leader (CCL) to provide individualized nursing care in a home setting. This role requires strong clinical reasoning, critical thinking, and leadership skills to ensure high-quality resident care.
Responsibilities:
- Utilize the nursing process (assessment, diagnosis, planning, implementation, evaluation) to deliver patient-centered care.
- Collaborate with inter-professional colleagues to plan, implement, and evaluate care.
- Document resident history, symptoms, medications, and care provided in compliance with regulations.
- Supervise and delegate tasks to unlicensed personnel as needed.
- Serve in leadership roles on departmental or organizational committees.
- Utilize technology to enhance quality of care.
- Coordinate care, conduct resident assessments, provide education, and triage as necessary.
- Ensure compliance with the Nurse Practice Act and professional nursing standards.
Qualifications:
Education:
- Bachelor’s degree in Nursing (BSN) preferred.
- Graduate of a nationally accredited nursing program (CCNE, ACEN, NLN CNEA) preferred.
Experience:
- Minimum two years of long-term care or geriatric nursing experience preferred.
- Experience with Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) preferred.
Licensure & Certifications:
- Active RN license in the state of practice or multistate licensure (NLC).
- Basic Life Support (BLS) certification required.
- Valid driver’s license may be required, depending on location.
- Must maintain required department-specific competencies and certifications.
Job Types: Full-time, Contract
Pay: $68.00 per hour
