The Nueces Center for Mental Health and Intellectual Disabilities (MHID) is dedicated to helping the residents of Nueces County who are living with mental illness and/or intellectual and developmental disabilities. We serve over 10,000 people each year with psychiatric and behavioral health care outpatient services, counseling, training, employment assistance, housing support, homeless outreach, children's mental health care and more.

Hourly Wage: $22.20
Full-Time, 40 Hour Work Week, Paid Bi-weekly
General Description Assists
persons with intellectual and developmental disabilities in developing their
Person Directed Plan (PDP), which identifies services & supports needed to
obtain life goals. Monitors consumers' progress towards goals & objectives
and satisfaction with services & supports available through the Home and
Community-based Services program. This
is completed through monthly face to face contact & frequent telephone
contact with the individuals assigned, their families, and their service providers.
The work requires intensive coordination of private providers' services and
available community resources including Medicaid, Social Security, and other
community based programs where available. Also requires interaction and
cooperation with various internal departments within Nueces Center for Mental
Health and Intellectual Disabilities. Extensive documentation is necessary to
ensure that the individual receives needed services and makes progress towards
their life goals. All documentation must be completed within required
timeframes specific to each program. Staff must be compliant with Center
policies and procedures, program protocols and requirements of other regulatory
bodies. Works under general supervision, with moderate latitude for the use of
initiative and independent judgement.
Position requires efficient time management. Will have a schedule that is flexible to the
needs of consumers served. Caseload is
approximately 42 individuals. Must have
reliable transportation. May use personal vehicle to conduct business. Must have a valid driver's license at all
times, liability insurance, and a good driving record.
Minimum Qualifications Minimum: A qualified
professional with appropriate clinical training and experience is defined as an
employee with a minimum of a bachelor's or advanced degree from an accredited
college or university
Preferred: Bachelor's or advanced degree from an
accredited college or university with a major in social, behavioral, or human
service field including but not limited to, psychology, social work, medicine,
nursing, rehabilitation, counseling, sociology, human development, gerontology,
educational psychology, education, and criminal justice; Has experience with
the IDD population and/or experience in direct care; Case management
experience; Customer service experience; Bilingual in English and Spanish
Knowledge, Skills, and Abilities
- Knowledge of individual needs/characteristics associated with intellectual and developmental disabilities.
- Knowledge of provider services and community-based resources.
- Knowledge of HCS and other waiver principles and billing guidelines
- Skill in individual needs assessment and treatment plan development and discharge planning. Ability to make appropriate referrals and linkages.
- Ability to advocate effectively for consumers.
- Ability to work independently, organize work effectively, and communicate clearly (verbally, in writing).
- Must be computer literate.
- Ability to develop productive relationships with individuals, family members and providers that focus on consumer specific outcomes.
- Knowledge of applicable rules, regulations and policies and procedures.
- In the course of being employed, understands and implements trauma informed care practices.
