What are the responsibilities and job description for the LPN | ADULT DAY HEALTH CENTER position at Nuestra Casa Adult Day Health Center?
Nuestra Casa Adult Day Health Center is looking for an LPN (Licensed Practical Nurse) to oversee both day-to-day nursing operations and quality of care provided, as well as the timely preparation of documentation that meets compliance guidelines with Massachusetts ADHC regulations as well as DPH requirements.
Responsibilities:
- Provision of nursing services and treatments to participants;
- Supervision of the care and services provided to participants by Program Aides, volunteers, and other personnel, as applicable;
- Participating in the completion of participants' comprehensive assessments and plans of care;
- Writing nursing notes at least one time per month or more often as necessary to address significant changes and health care teaching in each participant's health record;
- Responding appropriately to any acute changes in a participant's health or functioning;
- Providing appropriate health care education for each participant and legally authorized representative;
- Obtaining, reviewing, and implementing primary care provider orders; and
- Coordinating the participant's care with the primary care provider and, if applicable, community service providers.
REQUIREMENTS:
- A Program's licensed nurse(s) shall have at least two years of recent experience in direct care of adults or chronically disabled persons.
- Medium Work-Lifting 50 lbs maximum with frequent lifting and or carrying of objects weighing up to 25 lbs.
- A valid MA Nursing License.
- Pre-employment clearance by a physician that certifies the employee to be free of infectious disease and/or restrictions that would inhibit performing job related responsibilities
- Able to read, write and speak English and Spanish
