Job Overview
We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care, particularly in wound management, and will be responsible for assessing, diagnosing, and treating patients with various types of wounds. This role will require traveling to 1-2 long term care facilities per day. Mileage is reimbursed at 70 cents per mile.
Shift: Day shift, Mon-Fri
Pay: $125,000 depending on experience

Responsibilities
- Conduct comprehensive assessments of patients with wounds, including history taking and physical examinations.
- Develop and implement individualized care plans based on assessment findings and best practices in wound care.
- Perform wound care treatments, including dressing changes, debridement, and infection management.
- Collaborate with interdisciplinary teams to ensure holistic patient care and effective treatment outcomes.
- Educate patients and their families on wound care management, prevention strategies, and the importance of follow-up appointments.
- Document all patient interactions accurately in accordance with ICD-10 coding standards.
- Provide support for patients with behavioral health concerns related to their medical conditions.
- Maintain knowledge of current best practices in wound care and participate in ongoing professional development.
Experience
- A valid Nurse Practitioner license with a focus on wound care or related fields.
- Experience in patient care settings such as assisted living or working with individuals with disabilities is preferred.
- Proficiency in IV insertion and venipuncture techniques is highly desirable.
- Knowledge of physiology as it relates to wound healing processes is essential.
- Familiarity with infant care may be beneficial for specific patient populations.
- Strong communication skills to effectively interact with patients, families, and healthcare team members.
