What you'll do:
- Empower patient to take an active role in their care by encouraging adherence to medical advice and appointment attendance.
- Collaborate with clients and families to develop and adjust personalized care plans.
- Connect clients with community resources to enhance their well-being and quality of life.
- Maintain accurate and timely documentation in line with agency policies.
- One to two visits per month.
- Master’s degree in Social Work (MSW) from an accredited college.
- LSW or LCSW license in the state of Nevada
- Minimum of one year of supervised or equivalent work experience (home care experience preferred).
- Strong communication, organizational, and problem-solving abilities.
