Role : Care Coordinator
Location: Philadelphia, PA
Duration: Contract 13 weeks
Hours : 40 hr/week (5*8)
Pay range: $21/hr
Setting: Detention center
Requirements:
- Prior 1 years experience as Coordinator Required.
- Active BLS-AHA .
Roles and Responsibilities:
- Coordinate care for patients with multiple providers, monitor treatment plans, and ensure effective treatment plans
- Educate patients on their conditions, provide pre and post care instructions, and review medication compliance
- Communicate with patients, family, and staff, and establish relationships with health plan case managers and pharmacies
- Develop patient goals and monitor progress
- Prepare reports related to patient progress, including exam results, physician notes, and other related documents
- Participate in community outreach activities as determined by the organization's needs
