Overview of the Role :
Our client is seeking a compassionate and skilled Nurse Practitioner to join a dynamic team within the PACE Health Clinic and Day Center. In this impactful role, you will provide both direct and indirect care to participants, working under the guidance of the Medical Director / PCP. You will have the opportunity to manage complex clinical situations, facilitating high-quality care for a vulnerable population. Your contributions will directly support the health and well-being of elderly patients in a nurturing environment.
Responsibilities :
- Conduct health histories, physical exams, and patient teaching with precision.
- Order, interpret, and follow up on lab tests, ensuring comprehensive documentation in the EMR.
- Initiate emergency measures and identify abnormalities for immediate referral.
- Evaluate and adjust medication and treatment orders per established protocols.
- Coordinate and manage participant care daily in collaboration with the attending physician.
- Liaise with physicians, nurses, and healthcare professionals, fostering educational communication.
- Participate in the PACE provider call schedule for cohesive team support.
- Actively engage as a member of the Interdisciplinary Team and contribute to care planning.
- Document and communicate participant changes, ensuring informed team collaboration.
- Evaluate and treat participants in various settings, including homes and healthcare facilities.
- Mentor medical students and residents during their PACE rotation.
- Educate participants on Advanced Directives and discuss DNR orders with physicians as needed.
