- Provide all levels of care to the patients
- Support patients in Antioch, IL and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule
- Demonstrate experience with tablets, mobile phones, and EMR software
- Monday through Friday on the day shift; no weekends, on-call or holidays
- 2 years of experience with Home Health or Home Health Care
- 1 years of experience with Epic Home Health
- 1 years of experience with Electronic Medical Record (EMR) or Other EMR or TruChart EHR
- Valid Registered Nurse (RN) certification/license in Illinois (IL)
- High School/GED
- Valid Regular Driver License (DL) certification/license
- Are you okay with the required travel for this position?
