RN HOSPICE Care at Home (Saratoga/Washington/Montgomery County area) Per Diem
The Community Hospice has a wonderful opportunity for an RN seeking per diem employment to work as part of our Hospice Home Care Team -caring for patients and families residing primarily in Saratoga county area.
Needs would fall between Monday - Friday 8a-4:30p

Special Per Diem Rates based on monthly hours commitment
Experience based (non per diem) Pay Range: $35.00-$47.75
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location

- National pre-taxed mileage reimbursement
- Meetings and charting conducted via supplied lap top
- Comprehensive orientation provided Prioritizing Your Safety: Ensuring your safety is our top priority. From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.
- Coordinate with an interdisciplinary team, to develop a comprehensive plan of care to provide added support to patients and families in their homes. This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- The RN must be able to communicate effectively, verbally and written
- 1 year experience as an RN , preferably in Hospice/Home Care or acute care.
- Valid Driver's License and Reliable vehicle
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers Understanding of Hospice principles preferred
