Position: PRN Private Duty Nurse/Pediatric LPN - Shift Work

As a Private Duty Nurse, you will play a pivotal role in improving the lives of our patients and their families. You’ll provide high-quality, comprehensive nursing care while working closely with the interdisciplinary team to ensure patient-centered care.

Responsibilities

• Deliver skilled nursing care in the client’s home under the direction of a Registered Nurse (for LPNs) or autonomously (for RNs).
• Participate in the development and execution of patient care plans in collaboration with the interdisciplinary team.
• Provide health education to patients and caregivers to promote understanding and participation in care.
• Accept and document verbal and written orders from physicians, ensuring RN Case Manager co-signature when required.
• Communicate effectively with physicians, patients, families, and team members regarding patient concerns and changes in condition.
• Accurately document all care provided in a timely manner.
Follow up with physicians, patients, and caregivers as needed to address concerns and ensure continuity of care.

Requirements

• Active nursing license in the state of Connecticut or a compact license (LPN).
• Home health experience preferred
• Reliable transportation to perform job duties.
• Ability to lift, push, pull up to 50-100 lbs.

Pay is based off experience and is between $28 to $32.00
