Job Summary:
The Assistant Social Worker supports residents, families, and the Social Services team by assisting with discharge planning, case management, community referrals, and administrative tasks. This role ensures residents receive appropriate care, resources, and emotional support while maintaining compliance with facility and regulatory standards.
Key Responsibilities:
- Assist in psychosocial assessments and advocate for resident needs.
- Support discharge planning, coordinating transitions to home, another facility, or community care.
- Coordinate community referrals for home care, transportation, and mental health support.
- Maintain accurate case notes and documentation in compliance with regulations.
- Provide emotional support and crisis intervention as needed.
Qualifications:
- Education: High School diploma.
- Experience: Healthcare, long-term care, or rehabilitation experience preferred.
- Skills: Strong communication, organizational, and Computer Literacy
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
Expected hours: 30 – 40 per week
