Location: Ohio (USA) Schedule: Monday-Friday, Day Shift (40 hours/week)
Salary Range: $58,000-$78,000 annually
Key Responsibilities:
- Provide professional social work services in hospice care setting
- Conduct psychosocial assessments and crisis interventions
- Facilitate advance directive completion and guardianship processes
- Deliver therapeutic support and family counseling
- Collaborate with interdisciplinary healthcare teams
- Advocate for patient rights and self-determination
- Manage complex family dynamics and interventions
- Monitor length of stay (LOS) and resource utilization
Required Qualifications:
- Master's Degree in Social Work (MSW)
- Current Ohio Social Work license
- 1-2 years professional experience
- Valid driver's license and auto insurance
- Strong knowledge of Medicaid waiver programs
- Expertise in abuse/neglect indicators and protocols
