Key Responsibilities:
- Lead and direct stroke and sepsis programs.
- Manage patient care from door to discharge.
- Coordinate 90-day post-discharge follow-up to ensure continuity of care.
- Collaborate with physician partners and facility leaders to optimize clinical outcomes.
- Drive program development and marketing initiatives for increased community engagement.
- Provide education and mentorship to care teams, fostering a culture of continuous improvement.
- Oversee program budgets and operations, ensuring efficient resource management.
- Facilitate quality improvement initiatives to enhance patient safety and care standards.
- Support business planning and program optimization for sustained growth.
Required Qualifications:
- Current California RN license.
- Minimum Bachelor's degree in nursing.
- At least Experience in Acute care.
- Current BLS certification.
- Proven experience in stroke and sepsis program coordination.
- Strong leadership and communication skills, with a focus on teamwork.
