Overview
We are seeking a dedicated and organized Care Coordinator to join our team. The Care Coordinator plays a crucial role in managing and facilitating patient care in a home health care setting. This position involves coordinating services for patients to ensure they receive high-quality, personalized care in their homes. The Care Coordinator will collaborate with patient families, and caregivers to create and implement care plans that meet the patients' needs.
Responsibilities
The Care Coordinator is responsible for the day-to-day management of the agency, including but not limited to:
- Organizing and supervising the administrative function of the agency;
- Maintaining an ongoing liaison with the governing body, the professional personnel and staff;
- Employing qualified personnel and ensuring adequate staff orientation, training, education and evaluation;
- Ensuring the accuracy of public information materials and activities;
- Implementing an effective budgeting and accounting system;
- Maintaining compliance with applicable laws and regulations and implementing corrective action in response to reports of organization committees and regulatory agencies;
- Arranging and negotiating services provided through contractual agreement; and Implementing the policies and procedures approved by the governing body.
Qualifications
- An individual who has evidence of at least one year of training and experience in direct health care service delivery with at least one year within the last in home health care or a related health program.
- Exhibits leadership skills
- GED or High School Diploma
- Complete a minimum of six clock hours per year of continuing
- education in subjects related to the duties of the administrator.
- Be able to read write and comprehend English
- Not been convicted of a felony or misdemeanor
- Excellent organizational skills with the ability to manage multiple tasks simultaneously.
- Proficient in office software applications and electronic health record systems.
- Exceptional communication skills, both verbal and written, with a focus on customer service.
- Ability to work collaboratively within a team environment while also being self-motivated.
- Attention to detail is critical for maintaining accurate records and ensuring quality care.

Pay: $21-$25.00 per hour varies per experience.
