Outreach Coordinator

Job Summary:

The Cal-AIM Outreach Coordinator will primarily focus on enrolling eligible individuals into the California Advancing and Innovating Medi-Cal (Cal-AIM) program, ensuring they gain access to essential healthcare services and supports. The role involves conducting outreach to prospective members, guiding them through the enrollment process, and assisting with any follow-up actions necessary to complete their enrollment. The Outreach Coordinator will also foster relationships with local organizations, healthcare providers, and community leaders to encourage program participation and maintain community engagement.

Key Responsibilities:

• Enrollment Coordination:
• Lead the enrollment process for individuals eligible for the Cal-AIM program, ensuring smooth and timely processing.
• Assist individuals in understanding the benefits of the program and guide them through the application and enrollment steps.
• Ensure all required documentation is collected and submitted accurately for program enrollment.
• Follow up with individuals to ensure they have completed all necessary steps for enrollment and resolve any barriers to enrollment.
• Community Outreach:
• Conduct outreach activities to raise awareness of Cal-AIM and encourage participation in the program.
• Organize and attend community events, presentations, and informational sessions to engage prospective members.
• Build and maintain strong relationships with community organizations, healthcare providers, and other local partners to promote Cal-AIM.
• Program Education & Support:
• Provide clear and accurate information to potential members about the Cal-AIM program, eligibility, benefits, and services available.
• Answer questions, address concerns, and assist with troubleshooting enrollment challenges.
• Data Management & Reporting:
• Maintain accurate records of outreach efforts and enrollment statuses.
• Provide regular reports on the number of individuals enrolled and outreach activities.
• Track and document progress to meet program enrollment goals.
• Collaboration:
• Work closely with other departments to ensure smooth enrollment and support for participants.
• Collaborate with the marketing team to develop outreach materials and strategies.
• Feedback & Continuous Improvement:
• Gather feedback from enrolled members and community partners to improve the outreach and enrollment process.
• Suggest improvements to the enrollment process based on community needs and challenges.
