Job description
The Intake Coordinator/Scheduler helps to schedule and coordinate a multidisciplinary plan of care involving the treating therapist and patient/family in coordination with the physician’s orders and is also responsible for QA of therapy/HHA notes.
Principal Accountabilities:
- Verifies demographic, insurance and financial information from patient or caregiver.
- Tracks various aspects of care requirements including date of care delivery, orders, supervisory visits, certification periods and re-hospitalization.
- Communicates changes in orders to care delivery staff.
- Monitors and reviews field staff timeliness with documentation and reports findings to management.
- Communicates in an effective and professional manner with Physicians, ancillary departments, nursing units, physicians’ office staff, insurance companies, as well as patients and their families.
- Completes thorough and accurate documentation.
- Must be proficient in calling on doctors orders.
Responsibilities:
- Maintain remaining visit report to ensure the approved number of visits is not exceeded
- Review caseloads to ensure that the therapist is maintaining their visits
- Maintain database pertaining to therapist caseloads and patient information
- Assist Case Managers with calls pertaining to patient therapy issues
- Contact patients/therapists as requested by Case Managers and the Administrator to discuss missed visits
- Provide support to therapists in need of assistance with scheduling patients, communication with families, etc.
- Addresses and resolves conflict issues that arise between parent and therapist
- Provide detailed information to Case Managers regarding patient assignment as needed
- Provide detailed information to Case Managers in order to assist with the assignment of admissions, evaluations or discharges.
- Assist with updates to the therapy schedule upon request and approval
- Coordinates with families and therapists to ensure coverage for patients whose therapist is unavailable, planned or unplanned.
- Maintain caseloads as needed for frequency of visits by coordinating between the family and the therapist
- Assists additional scheduling coordinators as needed/requested by Case Managers .
- Constant communication with therapists to coordinate changes to caseload due to family or company request to include frequency of visits or special requests
- Maintain the supervisor/assistant list (supervisory visits every 30 days)
- Provide information on Weekly Calls Report as needed for accurate reporting
- Accurate documentation regarding communication in each patient chart

Qualifications:
- Home Health Scheduling required (with Medicare systems preferred)
- Candidates with 6 month to 1 year Home Health experience will be preferred
- Must possess and maintain good physical stamina and mental health
- Ideal candidate is self-directed with the ability to work with little supervision
- Good organizational skills
- Flexible and cooperative in fulfilling role obligation
Experience / Knowledge / Skills:
- 6 months to 1 years of experience in a Home Health office setting
- 6 months to 1 year of year experience as a scheduler
- Bilingual is perfered but not a deal breaker
