Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will have experience in Home Health Care, with a focus on providing high-quality patient care. The Registered Nurse will work in the patient's home in various conditions. Ability to work flexible schedules, travel locally.
Responsibilities
- Conduct comprehensive physical examinations and assessments of patients.
- Administer medications and treatments as prescribed by physicians.
- Conduct Start of Cares, Recertifications, SN visits.
- Implement/develop document for plan of care to ensure quality and continuity of care.
- Educate patients and their families on health management and treatment plans.
- Utilize Kinnser for accurate documentation and patient management.
- Collaborate with multidisciplinary teams to develop individualized care plans.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in Home Health is preferred.
- Familiarity with Kinnser or similar electronic health record systems is a plus.
- Knowledge of Oasis is preferred.
- Experience with wound care preferred.
- Strong communication skills, both verbal and written, for effective patient education.
- Ability to work collaboratively in a fast-paced healthcare environment.
