Job Summary:

Responsibilities:
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Conduct comprehensive assessments of patients' physical and emotional needs, developing personalized care plans to address these needs.
- Maintain accurate and detailed records of patient interactions, treatment plans, and outcomes.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care and achieve optimal patient outcomes.
Requirements:
- Licensure as a Registered Nurse (RN) in the state where the position will be located.
- A minimum of 1 year of experience in home health or a related field.
