We are looking for a Community Liaison to work Full Time 40 hours weekly, in this position, you will be responsible for new referrals to the Agency. Serve as the primary liaison to prospective patients and designated health care providers as well as insurance case management professionals and community groups to ensure safe and appropriate care for all new patients.

Duties And Responsibilities
- Establishes and develops relationships with staff, physicians, referral sources, patients and family members to facilitate the referral/admission process and acts as liaison between all entities. Explains the types of care and services the Agency offers to patients and their families.
- Recruits referrals and coordinates with intake Staff in assisting to coordinate admissions with the Agency’s daily workload
- Assesses incoming patients physical, social and psychosocial needs to ensure they meet the agency’s admission criteria. Consults with referral sources on complex patient discharges and coordinates patient’s care with agency. Obtains financial information, as necessary, from patient, family and patient chart.
- Establishes open lines of communication with referral sources to promote good community relations. Works to provide service to existing accounts and strives to increase referral numbers. Responds to questions, complaints and inquiries from patients, families, referral sources and community providers as appropriate.
- Participates in appropriate professional, community organizations and outreach activities.
- Reviews weekly NVNA Admission Reports, records and maintains on-going statistics, i.e., tracking referral sources, admissions and insurers and submits reports as requested/required.
- Bachelor’s degree preferred
- 2 years of Administrative experience
- Healthcare, Intake or sales experience preferred
