What Will You Be Doing
We are looking for an experienced, talented, and compassionate Home Health LPN to work full-time at our service areas South Shore area.
The Home Health LPN will work collaboratively with the RN case manager to provide skilled nursing services to patients in their homes. You are responsible for direct patient care and provide any status changes directly to the physician and case manager. You must work collaboratively within the team structure and have strong communication skills.

What We’re Looking For In You
- Current Massachusetts Licensed Practical Nurse license in good standing.
- 2 years of LPN experience required.
- Prior homecare and hospice experience preferred.
- Strong clinical assessment skills and the ability to prioritize and triage patients.
- Exceptional communication skills and the ability to work within a team structure.
- Ability to work independently and understanding of how to access resources for patients.
- Excellent computer skills.
