Key Responsibilities:
Intake and Enrollment:
• The Admissions and Assessment Coordinator handles the initial contact with potential clients and their families, coordinating and guiding them through the application process and gathering necessary information.
• Ensures smooth and efficient operation of the admissions process, from initial contact to program enrollment.
• Conduct preliminary assessments and gather necessary information from prospective clients and their families
Eligibility Determination:
• Review applications and supplemental documentation (e.g., assessments, IEPs) to determine eligibility for the organization's programs based on established criteria and regulations.
• Working knowledge of Person-Centered Planning processes and the identification of individualized needs per person served. This is to include the comprehension of what is important “to” an Individual as well as what is important “for” an individual as it pertains to the goal writing process. Additionally, a respect for cultural competence, diversity sensitivity, and ability to learn about a variety of needs of individuals served.
• To confirm that individuals have an active funding source which is accepted by NW Works.
Documentation Management:
• The Admissions and Assessment Coordinator ensures accurate and complete documentation is maintained for all applicants and clients.
• Preparation and verification of admission paperwork, including eligibility information and consent forms, medical information including proper scripts and prescriptions for medications.
• If an individual has a medical history, for example seizures, inclusion of seizure protocol or other medical requirements.
• Maintain accurate records of inquiries, assessments, and admissions in compliance with regulatory requirements.
Coordination and Communication:
• Work closely with other professionals, such as case managers and service providers, to facilitate the transition of new clients into the organization's services.
• Serve as the welcoming point of contact for inquiries from families, referral sources, and community organizations.
• Schedule and conduct facility tours for prospective clients and families, providing detailed information about services.
• Provide compassionate support to prospective clients and families, addressing questions and concerns throughout the admission process.
• Collaborate with the Directors to develop and implement strategies to enhance the overall admissions experience and onboarding success.
• In collaboration with Directors, establishes working relationships across all departments for the success of persons served as well as the Organization. Maintains a team approach to supporting individuals through curriculum and evidenced based practices through training, data analysis, and programmatic development.
Data Entry and Reporting:
• Responsible for entering client information into relevant databases and generating reports as needed.
• Track admissions statistics and maintain databases related to inquiries, referrals and admissions.
• Prepare reports as requested by the Directors to analyze trends and identify opportunities for improvement.
Admissions Process Facilitation:
• Establishes measurable goal data in conjunction with Directors and case coordinators as they are needed for the Person Centered Planning and Strategic Planning process of the program.
Other Areas:
• Develops and implements survey tools for satisfaction to administer for feedback from persons served.
• Develops implements and presents training and curriculums to clients, staff, and external stakeholders.
• Establishes working knowledge of regulatory requirements
• Work Readiness Assessment and Job Task Analysis
• Will take part in completing job task analysis on possible job openings with local business to see if we have any clients that match those skills for possible future placement.
Knowledge, Skills and Abilities:
• Strong organizational skills.
• Strong communication skills include effective communication with people in potentially stressful situations.
• Strong quantitative and analytical skills with process orientation.
• Collaborative and able to work in a team environment with multiple constituents, clients and customers.
• Knowledge of Medicaid Waiver Services, Workforce Innovations and Opportunities Act, and the Department of Aging and Rehabilitation Services for individuals with developmental disabilities.
• Knowledge of data collecting and process implementation.
Technical Skills:
• Effective use of Microsoft Office
• Ability to navigate mobile applications
• This role routinely uses standard office equipment such as laptop computers, phones, and photocopiers.
Minimum Education, Certifications and Experience:
• Bachelor's Degree in approved Human Services / Business field preferred or related experience with a High School Diploma or GED
• At least 2 years of experience working in training or teaching environment.
• Verification of a clear driving record
Work Environment:
