What are the responsibilities and job description for the Kinship Navigator position at NWF Health Network?
POSITION OBJECTIVE: To serve as primary contact and support for Kinship Caregivers during their navigation of the NWF Health Network System of Care. Assist families to meet their needs at the time of placement and initial the licensing process with families.
ESSENTIAL FUNCTIONS:
· Contacts family within 48 hours of receiving the referral to gather information regarding their needs and schedule a visit with the family.
· Completes in person visits with prospective Level I caregivers to assess needs and complete documentation for the licensing process.
· Provides back up support to the Kinship Program Administrative staff.
· Gathers required information need for Unified Home Study and ongoing assessment
.· Maintains weekly contact with all families on active caseload by means of home visits, office visits, phone contact or correspondence.
· Assist with DCF/ ACCESS applications with families to seek public benefits (cash assistance, food stamps and Medicaid).
· Completes ongoing referrals and assist with resources to the family.
· Follows-up on all referrals to determine if family received services.
· Maintains ongoing relationships with organizations, individuals and providers that assist the program, and renews and expands those relationships.
· Works with Licensing Specialist and Case Manager to determine any family needs and meet those needs.
· Ensures proper documentation of all activities in Florida Safe Families Network (FSFN) within 48 hours of activity.
· Develops and maintains a list of individuals, organizations and providers that may assist children and families (Businesses, Civic Groups, Schools, Neighborhood Associations and Faith Groups).
· Works as part of a multi-disciplinary team to meet the needs of children and families.
· Supports children living with relative caregivers to remain outside of foster care.
· Assists with planning and support for annual activities for children in foster care.
· Meets with Kinship Program Supervisor on a weekly basis for supervision to review cases, complete training, professional development, and program development.
· Participate in quality assurance reviews of the program services, outcomes and records.
· Other related duties as assigned.

QUALIFICATIONS:
REQUIRED:
· High School Diploma
PREFERRED:
· Multiple forms of child welfare or child welfare related experience
· Prior experience with foster home licensing
SKILLS:
· Knowledge of childhood development and related mental health processes.
· Knowledge of child abuse or neglect reporting procedures and methods.
· Knowledge of child and family histories, habits, tendencies, general behaviors, etc.
· Knowledge of agency's organizational structure, standard operating procedures, and policies.
· Ability to establish effective working relationships with clients, co-workers, supervisors and other individuals in a diverse arena.
· Ability to communicate effectively and professionally, verbally and in writing.
· Computer skills to include basic knowledge of FSFN, Outlook, Excel, and Word
· Ability to understand, familiarize and apply guidelines and procedures.
· Ability to work independently and utilize critical thinking skills, as well as initiative to maximize ability in meeting deadlines.
· Knowledge of community resources· Ability to work flexible hours as required· Ability to resolve conflicts in a professional manner· Ability to make home visits and to travel
