Position Overview: We are seeking a passionate and experienced HIV Patient Navigator to join our team. The ideal candidate will play a crucial role in supporting individuals living with HIV by providing guidance, advocacy, and resources to navigate the healthcare system. As an HIV Patient Navigator, you will work closely with patients referred to us from Brooklyn Hospital to ensure they receive the highest quality of care and have access to the necessary services and support.
Key Responsibilities:
- Patient Advocacy: Advocate for the rights and needs of HIV patients within the healthcare system, ensuring they receive timely and appropriate care.
- Case Management: Provide comprehensive case management services, including coordinating medical appointments, assisting with medication adherence, and connecting patients with relevant social services.
- Education and Support: Offer support groups and workshops on HIV-related topics, treatment options, and prevention strategies. Provide emotional support and empower patients to actively participate in their healthcare journey.
- Resource Coordination: Collaborate with healthcare professionals, community organizations, and social services to connect patients with relevant resources, support groups, and financial assistance programs, and other community-based resources.
- Community Outreach: Engage in outreach activities to build and maintain partnerships with community organizations.
- Data Tracking and Reporting: Maintain accurate and up-to-date records of patient interactions, interventions, and outcomes. While adhering to strict confidentiality standards in handling patient information and ensure compliance with HIPAA regulations.
Qualifications:
- Associate degree in social work, public health, or a related field (Bachelor's preferred).
- Previous experience in HIV/AIDS advocacy, case management, or patient navigation.
- Knowledge of HIV/AIDS medical, social, and psychological aspects.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively in a multidisciplinary team.
- Compassion, empathy, and a commitment to serving the needs of individuals living with HIV.
