The MDS Coordinator Assessor is responsible for ensuring the accurate and timely completion of the Minimum Data Set (MDS) assessments in compliance with federal and state regulations. This individual will work closely with management, nursing staff, and other healthcare professionals to ensure residents' highest quality of care. The successful candidate must hold a Licensed Practical Nurse (LPN) license.
Key Responsibilities
- Conduct MDS Assessments : Perform comprehensive assessments for residents, ensuring all data is accurate and submitted promptly.
- Coordinate Care Plans : Develop and update individualized care plans based on MDS assessments in collaboration with the interdisciplinary team.
