A Registered Nurse (RN) is responsible for creating and managing a patient's individualized care plan, which outlines the nursing interventions needed to address their unique health needs and goals. The RN Care Plan Nurse is the primary healthcare provider who develops and oversees a resident's care plan. The Care Plan Nurse must have managerial or supervisory experience. Other duties include the following :
Assessment – The care plan Nurse is responsible for gathering information about a resident's health history, current symptoms, and other details to develop a comprehensive picture of their health.
Planning—The care nurse will collaborate with the DON to outline the specific care and needs of the patient. To implement the plan, the care nurse leads care plan development and collaborates with other healthcare professionals, including DONs, doctors, LPNs, and nursing assistants.
Communication- Designing a care plan that serves as a communication tool between nurses allows them to ensure the same quality of care when they change shifts. Documentation- must be proficient in care planning documentation and guidelines.
Supervision- must have at least one year of experience supervising Care Plans, LPNs, and certified nursing assistants.
Qualifications
- Valid RN license for NYS
- Proven experience as a registered nurse
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures
