Job Description

Purpose:

Under the supervision of the Director of Nursing, the Assistant Head Nurse will be competent in pressure injury prevention, evaluations, treatments, and documentation according to hospital protocol. He/she will also be competent in the care of other wound types such as diabetic, venous, arterial, and surgical wounds. Serves as a resource of nursing care for wound care on an ongoing basis. Demonstrates leadership that is self-directing, creative, and innovative, with an ability to function effectively in stressful situations.

Specific Duties and Responsibilities:

The Wound Care Assistant Head Nurse is charged with and responsible for:

  • Demonstrates knowledge of pressure injury staging utilizing the National Pressure Injury Advisory Panel (NPIAP) guidelines.
  • Demonstrates knowledge of basic principles of wound healing and management.
  • Follows hospital pressure injury protocol for identification, management, and treatment of pressure injuries.
  • Complies with established documentation requirements in the electronic medical record (EMR), per hospital protocol.
  • Ensures and communicates preventive measures for pressure injury development.
  • Serves as a resource for best practices in wound healing to clinical team including nursing, medical and nutritional teams.
  • Evaluates assigned patients’ skin within 24 hours of admission.
  • Demonstrates knowledge and skill in handling approved electronic devices for photography (Rover).
  • Maintains patient privacy and dignity while performing a digital image.
  • Identifies the patient utilizing the two identifiers (name and medical record number).
  • Maintains patient safety by adhering to patient identification protocol by ensuring that an ID label including the patient’s initials, the medical record number, and the current date is present on the image.
  • Makes necessary recommendations to providers for referrals to other specialties.
  • Maintains communication by keeping nursing leaders, providers and DON informed of all newly developed cases.
  • Reports promptly to clinical interdisciplinary team, MD/RN, for treatment modifications.
  • Recommends specialty beds as appropriate for high-risk patients.
  • Keeps abreast with current evidence-based practices.
  • Participates/provides educational activities i.e.: orientation of nursing personnel and/or in-services as indicated.
  • Supervises nursing activities related to patient care. Enforces standards and procedures for accurate reporting and recording of symptoms and progress.
  • Provides guidance, counseling, and direction to staff in accordance with the organization and department goals and objectives.
  • Works with the interdisciplinary team to coordinate the management of patient care.
  • Assists in planning, directing, providing, and evaluating patient teaching.
  • Assures proper utilization of supplies and equipment; orders supply as needed.

Minimum Qualifications

For individuals holding an unrestricted registered professional nursing license BEFORE July 1,
2020 in New York State, another state, or United States territory:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse
in New York State; and
2. Holds, or obtains through facility orientation, valid and current certification in Basic Life
Support (BLS) through the American Heart Association (AHA); and
3. Six (6) months of experience as a registered nurse.
For individuals issued a registered professional nursing license on or AFTER July 1, 2020,
based on the expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the
Laws of 2018:
1. A valid New York State license and current registration to practice as a Registered Professional
Nurse in New York State; and
a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degrees, as identified by NYS
Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of
Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or
university, registered with the NYSED; or
b. Obtains a Bachelor’s of Science Degree in Nursing or higher, as described in “a” above, within ten
(10) years of the date of initial licensure; and
2. Holds, or obtains through facility orientation, valid and current certification in Basic Life
Support (BLS) through the American Heart Association (AHA); and
3. Six (6) months of experience as a registered nurse.
