Job Description
The Social Worker II, LMSW, LCSW is charged with and responsible for:

• Interview patients and / or relatives in order to provide psychosocial assessment, to determine nature and source of problems and their inter-relationship with medical problems under study or treatment.

• Provide casework services as well as supportive and crisis intervention counseling, to patients and families for discharge planning.

• Confer with Medical, Nursing, and other professionals, in multidisciplinary rounds, to develop an assessment and care planning towards a safe and timely discharge for the assigned patients caseload.

• Communicates with community agencies about provision of services. Initiates referrals on behalf of patients.

• Keeps weekly statistics of case activities.

• Recording of case material, and documentation in the Electronic Medical Record (EPIC), according to established guidelines and policies of CMS and the Department.

• Access to EPIC, Allscripts, Outlook, ParachuteHealth.

• Will be responsible for providing assessment and intervention on Rape, Domestic Violence, Elder Abuse, Child Abuse and Crisis Intervention.

• Will have rotational Emergency Room/Maternity/Pediatric Units, Inpatient Discharges coverage to include Weekends, Evenings and Holidays.

• Will participate in Social Workers weekend and holidays coverage for ED, Maternity and Pediatric on continues basis.

• Will participate in Performance Improvement activities and monthly meetings and will be able to become a resource and role model for junior staff and caseworkers in practicing Social Work in Health Care Environment.

• After cross training the Social Worker shall be able to cover all outpatient and inpatient units and Social Work programs within the hospital and its satellite clinics, to include Maternity/Pediatrics, Emergency Room, and training programs, etc.

Minimum Qualifications
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or

2. Valid license as an LCSW issued by the NYSED.
