Job Summary:
We are seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to join our home health team. The COTA will work under the supervision of a licensed occupational therapist (OT) to provide rehabilitative care to patients in their homes. This role involves implementing therapy plans, assisting patients in regaining independence, and promoting overall wellness.
Key Responsibilities:
- Provide direct occupational therapy services to patients in their homes as outlined in the treatment plan developed by the OT.
- Assist patients in performing therapeutic activities to improve mobility, strength, coordination, and daily living skills.
- Educate patients and caregivers on therapeutic exercises, adaptive equipment, and home safety modifications.
- Monitor patient progress and report findings to the OT to adjust treatment plans as necessary.
- Document patient care activities and progress notes in compliance with regulatory and company guidelines.
- Communicate effectively with the healthcare team to ensure comprehensive patient care.
- Adhere to all home health policies, procedures, and safety standards.
Qualifications:
- Certification as an Occupational Therapy Assistant (COTA) in the state of Nebraska.
- Prior experience in home health, geriatrics, or rehabilitation is preferred.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and adapt to different home environments.
- Reliable transportation and a valid driver's license.
