RN Case Manager - On-Call Weekender - PRN 1st Shift

Position Summary

In this role you will:

• Conduct assessments and complete client visits

• Coordinate care alongside therapies & client/families and coordinate, supervise, monitor, and develop care.

• Communicate with physicians

• Perform teaching to clients and families

• Complete complex nursing tasks within the home setting
• Handle case assignments that fluctuate, with typical caseload notifications by Friday.

Qualified candidates will have:

• Possess a valid RN License from the appropriate state. This shall include required CEU’s
  

• Minimum 2 years of nursing experience

• Oasis experience (Preferred – will provide training)

• Home Health experience required
