This position oversees admissions for long-term care, provides backup support for rehabilitation admissions, and ensures smooth transitions for residents. The coordinator is responsible for daily rounding and care planning, as well as handling non-coverage notices (NOMNC) and advanced beneficiary notices (ABN).
Additionally, this role includes psychosocial interventions, Medicaid applications, family engagement, and risk review meetings. The Resident Support and Case Management—Long Term Care also participates in resident councils, safe transition meetings, and interdisciplinary team discussions to enhance the quality of life for residents. The role requires close collaboration with nursing staff, therapy teams, and families to provide individualized support and advocacy.

Key Responsibilities:
Admissions Transitions:
- Manage long-term care admissions and serve as a backup for rehab admissions
- Conduct bedside visits as needed for new admissions
- Oversee safe transition meetings to ensure smooth resident transitions between care levels.
- Perform daily rounding to assess resident well-being and address concerns.
- Facilitate care planning meetings and Medicare A/B meetings
- Issue non-coverage and advance beneficiary notices (ABN) as required.
- Complete Minimum Data Set (MDS) assessments related to social services.
- Conduct intentional 1:1 visits with residents to provide psychosocial support.
- Implement psychosocial interventions, coordinating closely with nursing and therapy teams.
- Attend family socials to encourage engagement and open communication.
- Attend and support Resident Council meetings to advocate for resident needs.
- Assist families with Medicaid applications and financial guidance.
- Participate in risk review meetings to identify risk and early intervention opportunities.
- Contribute to daily team standup meetings and Social Services stand-downs for coordination.
- Rotate as Manager on Duty every 12 weeks, ensuring leadership presence.
Education:
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field.
- Previous experience in long-term care, skilled nursing, or healthcare social services is preferred.
- Strong knowledge of Medicare, Medicaid, and long-term care regulations
- Ability to provide emotional support and psychosocial interventions
- Excellent communication, problem-solving, and organizational skills
- Proficiency in care planning, documentation, and MDS processes
- Ability to work collaboratively with nurses, therapists, and families
