Position Summary:
We have an exciting opportunity to join our team as a Counselor.
In this role, the successful candidate The Counselor is responsible for providing case management support for participants of the Adult Education program and their families This includes screenings for Social Determinants of Health referral advocacy and benefits outreach eligibility screening and support with application and recertification processes As part of the Adult Education team the Case Manager works closely with instructors to identify needs They also provides educational workshops for participants on various topics of interest Excellent engagement skills and ability to work with individuals and families in crisis
Job Responsibilities:
- Interviews program participants to assess needs for social services
- Identifies and facilitates access to a variety of public and community agencies to address participants needs including the provision of food shelter employment clothing and other services
- Provide advocacy case management and support for SNAP eligibility screenings applications and re certifications as well as for other benefits nbsp;
- Schedule client appointments and acts as client advocate in obtaining services
- Manage caseload prepares narrative and statistical reports and documents correspondence regarding client status
- Facilitate access to and scheduling of GED testing for eligible participants
- Work closely with Adult Education instructors to identify individual and community social service needs
- Conduct outreach and educational workshops for participants
- Participate in program registrations and orientations as well as staff meetings required trainings and other appropriate meetings
- Document and track work including referrals SNAP applications outreach and case management
Minimum Qualifications:
To qualify you must have a High School Diploma or equivalent required Bachelor s Degree preferred 1 years relevant experience working ability to plan and manage Knowledge of public benefits History of immigrant community engagement

Good interpersonal skills Attention to detail

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $21.09 - $24.46 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
