What are the responsibilities and job description for the Intake Coordinator position at O'Brien House?
The mission of the O’Brien House Intake Care Coordinator within the Treatment department is to process, schedule and ensure all Clinical needs of clients are complete, such as: screening, admitting, life skills, referrals, educational assessments, treatment planning, employment, discharge, etc.
As the SRO Case Manager this person serves as a liaison between the SRO tenants managed by O’Brien House and the Louisiana Balance of State Continuum of Care/Louisiana Housing Corporation, who provides federal grant money through HUD for case management.
Education/Experience Minimum Requirements:
An Intake Care Coordinator must meet the following qualifications:
Master’s degree in social services discipline; or bachelor’s degree in a social services discipline with a least one-year relevant experience; or High School diploma with at least (4) years relevant experience.
Intake Coordinator will work with the Clinical Director prior to admission to ensure the consumer is properly linked to the appropriate levels of care by pre-screening applications. An intake/care coordinator should not be assigned as the client primary counselor.
