OPM Healthcare Services Inc. is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As a key member of our healthcare staff, the LPN will play a crucial role in providing comprehensive care and support to elderly individuals, ensuring their well-being, and enhancing their quality of life.

Key Responsibilities:

1. Clinical Assessment

   - Conduct thorough physical examinations for elderly residents.

2. Treatment Planning and Implementation:

   - Develop and implement individualized care plans in collaboration with the interdisciplinary healthcare team.

3. Chronic Disease Management:

   - Monitor and manage chronic health conditions prevalent in the elderly population, such as diabetes, hypertension, and arthritis per physician orders.

   - Educate residents and their families on disease management and lifestyle modifications.

4.  Collaboration and Communication:

   - Work closely with other members of the healthcare team to coordinate care.

   - Communicate effectively with residents and their families, fostering a supportive and collaborative care environment.

5. Documentation and Recordkeeping:

   - Maintain accurate and up-to-date medical records in compliance with regulatory standards.

   - Document resident progress, interventions, and outcomes to facilitate continuity of care.

Qualifications

Requirements:

1. Active and unrestricted LPN license.
2. Experience in elder care is preferred but not required.
3. Excellent communication and interpersonal skills.
4. Compassionate and patient-centered approach to care.
5. Ability to collaborate effectively with a multidisciplinary healthcare team.
6. Commitment to maintaining confidentiality and upholding ethical standards.
7.  Strong critical thinking and problem-solving abilities are a plus.

Salary : $19 - $27
