Key Responsibilities:
1. Clinical Assessment

- Monitor and manage chronic health conditions prevalent in the elderly population, such as diabetes, hypertension, and arthritis per physician orders.
- Educate residents and their families on disease management and lifestyle modifications.
4. Collaboration and Communication:
- Work closely with other members of the healthcare team to coordinate care.
- Communicate effectively with residents and their families, fostering a supportive and collaborative care environment.
5. Documentation and Recordkeeping:
- Maintain accurate and up-to-date medical records in compliance with regulatory standards.
- Document resident progress, interventions, and outcomes to facilitate continuity of care.
