Responsibilities
Social Services Director Essential Functions
- Overseeing the facility’s Social Services Department and ensuring that the needs of the residents are met on an individual basis
- Interviewing resident/families to obtain social history and involving the resident/family in planning objectives, goals and social services programs
- Assisting in the development of a written plan of care for each resident that identifies the problems/needs, the goals to be accomplished, and the recommended approach for each
- Interpreting social, psychological and emotional needs of the resident to the medical staff, attending physician and other resident care team members
- Arranging psychological care and evaluation services as needed
- Assisting in the scheduling of care plans and assessments to be presented and discussed at each committee meeting as needed
- Ensuring that all social services personnel are aware of the care plan
- Reviewing nurses notes to determine if the care plan is being followed
- Chairing the Behavior Management Program

Qualifications
Social Services Director Qualifications
- Must possess a Bachelor’s Degree from an approved school of Social Work (BSW, MSW, CSW’s are preferred) or a Bachelor’s in a human services field including but not limited to sociology, special education, rehabilitation counseling and psychology
- A minimum of one year of supervised social work experience in a health care setting working directly with patients
- A Social Services Designee form the state of employment if the individual does not possess a Bachelor’s Degree
- Have a thorough understanding of the principles of best Social Services practices
