Job Responsibilities: The Internal Medicine Nurse Practitioner/ Physician Assistant (NP/ AP) will be crucial in delivering exceptional, patient-centered care to our adult and geriatric patients in skilled nursing facilities. This position involves:

· Collaborate with the facility care team and healthcare providers to develop individualized, evidence-based treatment plans

· Diagnose medical conditions, prescribe medications, perform diagnostic and therapeutic procedures, and coordinate comprehensive care as required

· Maintain timely, accurate, and detailed documentation of patient assessments, diagnoses, and treatment plans in compliance with regulatory standards

· Promptly communicate changes in patient conditions and updates to treatment plans to the interdisciplinary clinical team

· Engage with patients, families, and facility staff to clearly explain care plans, interventions, and health outcomes, ensuring understanding and trust

· Build and sustain strong, compassionate relationships with patients, families, and staff to enhance care quality and support a collaborative care environment

Qualifications: 

· Nurse Practitioner or Physician Assistant license

· Valid Wisconsin APNP or PA-C license in good standing

· Valid DEA 

· BLS certification

· Minimum 1 year of experience as an NP or PA provider (preferred)

· Experience working in skilled nursing facilities or similar healthcare settings (preferred)

· Excellent diagnostic, clinical, and interpersonal communication skills

· Ability to work autonomously and collaborate effectively within interdisciplinary teams
