The Field Team RN will provide comprehensive nursing services to the Migrant and Seasonal Farm Worker patient population of the practice assigned to within Oak Orchard Health (OOH). The Field Team RN will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician, advanced practice practitioners, and other members of the multi-disciplinary team. The Field Team RN will work to their full scope of practice within the field. This position will require Tuesdays 3p-8p and Thursdays 3p-8p with options to pick up additional hours either supporting additional PES work or in the Oak Orchard primary care offices.
Job Responsibilities :
Assess and Screen in the field MSFWs needing triage :
Determine necessity of in-office medical, dental, vision, behavioral health needs based on the presenting information / nursing assessmentAnswer patient questions and provide education within scope of licensureAdminister vaccines / medications under standing orders of physician.Maintain up to date standing order(s) with assist of medical provider(s).Maintain and clean mobile unit between each patient as necessaryComplete point of care testing as allowed.Provide outreach to farms and schedule visits in collaboration with the Field Team lead.Maintain stock of vaccines and supplies for the field team.Delegate tasks and work as a patient care team with Field Team LPN1) Documentation
Maintain accurate medical records.Complete documentation as events occurs.Maintain inventory and ordering of field team medications and medical supplies.Compile and submit monthly reports regarding services rendered.2) Assist in population health efforts focusing on preventive care and chronic condition management :
Utilize population health software and / or electronic medical record to create and provide reminders for health screenings-including A1C, PHQ-9, Smoking cessation, etc.Provide education in person regarding the importance of health screenings and health maintenance appointments3) Participate as a member of the Care Team :
Foster a working relationship with patients, providers, patient engagement services staff and practice staffWork effectively with others to coordinate patient and access care support servicesProvide input relating to changes that may enhance the practice effectivenessParticipate in meetings and huddles as appropriateProvide feedback to providers regarding patient progress and barriers encountered4) Other Clinical Support RN Functions and Responsibilities :
Provide culturally competent care to all segments of our patient populationPromote health literacy with all patient interactionsIdentify opportunities to improve processes and servicesShare issues that are obstacles to meeting patient needs with practice and leadershipMaintain equipment supplies, verification of expiration dates, ordering.Perform other duties as assignedRequirements :
Skills / Qualifications :
Excellent communication skills and ability to form collaborative partnerships across all service settingsStrong attention to detail and precise documentation skillsStrong assessment skills, education skills and confidence working independently.Working knowledge of the provision of health care in a variety of settingsKnowledge of community resources preferableAbility to engage and build relationships with patientsAbility to prioritize workload and assess the need for collaboration with the health care teamBasic computer literacy skills, with competence in Microsoft Office products such as Work, Excel, Outlook and PowerPoint, and experience working in an electronic health record (EHR)Ability to assimilate new information and technologies into daily workBilingual desired but not requiredCommunity focused and compassionate to the needs of the agricultural community.Tuesdays 3p – 8p and Thursdays 3p – 8p with additional opportunity to pick up hours with the field team or supporting the Oak Orchard Primary Care offices.Education and Experience :
Graduate of an approved school of nursing with RN license to practice in NYS. Must maintain a current and valid registration certificate2-3 years’ experience in Primary Care, inpatient or outpatient Internal Medicine / Family Medicine, Public Health, or related fieldCurrent certification in Basic Life Support for Healthcare Providers or equivalentOrganization Wide Responsibilities :
Coordinate Vaccine ordering with Brockport and Albion vaccine coordinators to ensure that adequate supplies remain available.Ensure proper supplies ordered and available to the team.Participate in OOH efforts to provide the highest quality patient care to all segments of the communities we serveHandle confidential information in accordance with HIPPA as well as state and federal privacy and confidentiality rule Comply with organizational policies and proceduresAttend team meetings, trainings, ongoing skill development events and other functions as required
