Requirements
Other Requirements:
- Licensure as Registered Dietitian in the State of Wisconsin Required
- 2 years’ experience in long-term care preferred
- Local travel required
- Excellent interpersonal, communication and organizational skills
- Position may require unscheduled overtime, occasional weekend
- Must be willing and able to work flexible hours
- Provide medical nutrition therapy using evidence-based medicine to optimize patient care to our three skilled nursing facilities in Madison and Janesville
- Conduct nutrition interviews, complete nutrition assessments, formulate nutritional care plan, implement interventions, and provide education to improve nutrition quality of life
- Monitor and evaluate patient care and outcomes on an ongoing basis
- Document nutritional assessment using MDS 3.0
- Directs nutrition care of special needs population to ensure nutrition needs and customer satisfaction levels are met
- Collaborates and communicates with culinary and clinical departments on established processes, special diets, and menu requests, cleaning, and sanitation
- Ensures compliance with all federal, state, and local, policies and procedures, quality assurance, safety, operations, and personnel) by keep abreast of the most recent nutrition-related trends, research, policy, certifications, and standards of practice.
- Review and understand the Long-Term Care Survey Process and be knowledgeable of state and federal guidelines applicable to long term care
- Conduct rounds, audit medical records to determine facility compliance with state and federal guidelines
- Review quality initiative data and assist facilities in development and implementation of corrective action plans as needed
- Develop new education material, training, processes, and classes, as necessary
- Serve on, participate in, and attend various committees of the facility as required, and as appointed by the Administrator
- Assists in providing training, work direction and/or problem solving for other workers
