As an LVN at Oak River Rehabilitation Center, you will play a vital role in delivering high-quality patient care and contributing to our mission of providing exceptional rehabilitation services. Key Responsibilities :
- Provide direct patient care under the supervision of a Registered Nurse (RN) or physician.
- Administer medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
- Monitor and document patient progress, including vital signs, symptoms, and responses to treatment.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Educate patients and their families about health conditions, treatment options, and self-care techniques.
- Assist with daily living activities and ensure a comfortable and supportive environment for patients.
- Respond to patient concerns and changes in condition promptly and effectively. Qualifications :
- Valid LVN / LPN license in the state of CA
- Current BLS / CPR certification.
- Previous experience in a rehabilitation or long-term care setting preferred.
- Strong clinical skills with the ability to work both independently and as part of a team.
- Excellent communication and interpersonal skills.
