Description : Job Summary :
The Housing Coordinator enables people to apply for, and move into affordable housing within the community, as well as providing services that support a person to maintain living in their home.
Supervisory Responsibilities :
Duties / Responsibilities :
Assist clients in locating suitable housing in the communityComplete person-centered housing planAssist client in locating community resourcesPerforms outreach to the community and other appropriate agencies regarding HousingCommunicates regularly with clients and their support teams to discuss progress, problems, and plansTake clients to tour potential housing sitesAssist clients in completing housing applicationAssist with packing and move coordinationFollow up on a regular basis once move is completed to assist in housing stabilityAdvocating with community resources to prevent eviction when housing is at risk and maintain a person’s safetyIdentifying and intervening to prevent and / or manage behaviors that may jeopardize continued housingProviding in-service transportationWorking with public benefits, income opportunities, and landlord relationships in order to promote housing retentionMaintain progress notes as requiredAttend weekly team meeting and one on one meetings as requiredBenefits :

Required Skills / Abilities :
