The Outreach Specialist manages the care provided to individuals, helps them gain access to needed services and teaches skills necessary to achieve their goals.

The PATH (Projects for Assistance in the Transition from Homelessness) Outreach Specialist helps unhoused adults in our community live healthier and more fulfilling lives. The Outreach Specialist manages the care provided to individuals, helps them gain access to needed services and teaches skills necessary to achieve their goals.
Primary Responsibilities
- Connecting with unhoused individuals in the community
- Determining the need for services (medical, social, education, etc.)
- Developing a specific treatment plan that supports the client’s goals
- Helping the client obtain needed services, such as food or housing assistance, Medicaid, Medicare, Social Security benefits, transportation, workforce training, case management, etc.
- Advocating for the client and serving as a link between the court, healthcare providers, employers, community agencies and other Oaklawn services
- Other duties as assigned
Minimum Hiring Requirements
- High School Diploma/GED or Associate degree .Starting pay rate is $20.88.
- BA/BS or Masters degree. Starting pay rate is $22.34.
- Two years’ experience in a social service setting preferred (mental health preferred; experience with seriously mentally ill adults highly preferred)
- Must have a valid driver’s license and proof of insurance
