What You’ll Do
Assist Open Access Center clients with care navigation:
- Focus specifically on high-risk referrals
- Offer additional support and engagement to connect them to needed services
- Will complete associated paperwork, per policy and procedure.
- Assist with acute and intensive services for individuals who may require immediate intervention
- Provide a means assessing acute mental health crisis by phone or in person
Minimum Hiring Requirements:
- BA/BS in social service or a human services-related field preferred
- Two years’ experience in a social service setting preferred
- Must have a valid driver’s license
