What You’ll Do as a Family Resource Specialist:
Duties include:
- To provide case management services to clients, families and children which consists of services that help clients gain access to needed medical, social educational and other services.
- Provide direct assistance in gaining access to services, coordination of care, oversight of the entire case and linkage to appropriate services.
Case management services include:
- Assessment of the eligible individual to determine service needs
- Development of a specific care/treatment plan
- Referral and related activities to advocate and assist the individual obtain needed services
- Monitoring and follow up of the plan
- Evaluation (of impact of treatment plan on client’s recovery)
- Linkage and networking with DCS, court, schools, health care providers, employers and other community agencies
- Provide integration and on-going assistance to clients and families to help sustain within the community/least restrictive environment
- Accountable for the flow of information within a multidisciplinary team setting
- Supervise court ordered and/or treatment directed supervised visits with approved family members and support persons
- Provide integration and on-going support to youth and caregivers to help sustain relationships and positive transition experiences
- Special Populations/Skills/Employment duties, as assigned.
The Child / Adolescent Family Resource Specialist also:
- Interacts directly with clients and families. The frequency varies with needs, availability of client, and level of intensity. It is usually weekly or biweekly and includes availability for crisis intervention.
- Interacts frequently with outpatient therapists, physicians, and other center employees involved in the case.
- Has regular interaction with community agencies and providers such as: DCS, court, schools, health care providers, referrals and coordinated services.
- Caseload expectations will take into account training; case complexity, level of frequency and/or other assigned duties.
Minimum Hiring Requirements:
- BA/BS in social work, psychology, or related discipline required. MA/MS/MSW in social work, marriage and family, clinical mental health counseling, or education (counseling and human services only); degree must be licensable (LMFT, LMHC, LCSW).
- Two years’ experience in a social service setting preferred.
- Valid driver's license and proof of insurance
