What are the responsibilities and job description for the Residential Case Manager position at Oaks Integrated Care?
Now Hiring Residential Case Manager, to work within our adult residential group homes located throughout Camden County and Southern New Jersey. The residential programs provide services to mentally ill adults who may also have a developmental disability.
Schedule: Monday to Friday: 8:15 a.m. to 5:00 p.m. – meals shared with consumers
Responsibilities:
· Provide individual and group counseling and support;
· Provide activities to address daily living skills;
· Provide recreational and socialization activities;
· Provide services to families such as referral, advocacy, and service linkages;
· Participate in the development, review, and updating of individual treatment plans and comprehensive assessments;
· Participate in the development of discharge plans and making needed referrals;
· Assist youth directly to address self-care needs;
· Participate in case conferences;
· Provide support to auxiliary staff, student interns, and volunteers;
· Assist in the development of staff orientation programs;
· Maintain clinical documentation
· Perform related studies as needed and appropriate to the provision of partial care services.

Qualifications:
- Bachelor's (BA/BS/BSW) degree from an accredited college in a mental health related discipline;
- One (1) year of related work or life experience if the four year college education is not in a human service field or a combination of one or more years of college plus work or life experience; equal to four years;
- Registered Nurse (RN) license;
- Licensed practical nurse (LPN) with (2) years of related work or life experience;
- High School diploma or equivalent plus four years of related work or life experience
All candidates must possess a valid driver's license in good standing
