BASIC FUNCTION
The Physical Therapist supports the direction and supervision of physical therapy programs for the facility. Responsibilities include evaluations, assessments, program planning, implementation, follow-up, and case management to maximize each resident’s ability to function independently in their activities of daily living.
This Physical Therapy Assistant is accountable to the Director of Therapy.

PHYSICAL THERAPIST ESSENTIAL RESPONSIBILITIES (including but not limited to)
Assess resident’s needs; selects, administers and interprets appropriate evaluation procedures.
- PT Supervises the planning and implementation of individual resident physical therapy treatment programs. Assures treatment programs are modified as needed.
- Serves as a consultant and provides in-services to facility staff.
- Provides services in the following settings: skilled nursing facility, Assisted Living, Independent Living, Home Health.
- Works independently with little supervision and is sensitive to the needs of many constituents.
- Ability to plan, organize, and prioritize work including navigation amongst multiple billing, documentation, and communication systems.
- Maintains appropriate documentation per regulatory guidelines. Prepares and submits all clinical billing in a timely manner.
- Ensures adequate inventory of physical therapy supplies and equipment within budget guidelines with guidance and approval of Director.
- Ensures representation at resident related meetings as well as various committees as assigned.
- PT rovides leadership in further development/expansion of physical therapy services and projects to grow physical therapy programs for Oakwood.
- Communicates physical therapy philosophy and treatment approaches and goals to residents, families, and other members of the interdisciplinary team.
- Meet the annual goals and objectives that are consistent with the values and mission of Oakwood Village and continuous quality improvement.

- Attends in-services and approved continuing education programs for current best practices and optimal utilization of license potential.

- Willingness to work a flexible schedule including weekends and holidays as needed. Work hours vary, dependent upon patient need and caseload.

- Maintains Physical Therapy (PT) license by executing continuing education as required by State of Wisconsin; able to present evidence of licensure renewal materials as needed promptly.
PHYSICAL THERAPIST ESSENTIAL QUALIFICATIONS
- Excellent oral and written communication and human relation skills.
- Works independently with little supervision and is sensitive to the needs of many constituents.
- Ability to provide leadership that fosters a team approach in meeting the mission of Oakwood.
- Working knowledge of reimbursement sources; Medicare A (PPS), Medicare B, Medicaid, and Managed care insurance plans.
- Ability to maintain and protect confidentiality of information.
- Exhibit a desire to work with older adults and understand the aging process.
EXPERIENCE, EDUCATION, AND/OR TRAINING
- Graduate of an accredited university with a degree in Physical Therapy.
- Current licensure in the State of Wisconsin.
- Minimum of two years’ experience preferred.
- Long-term care experience preferred.
- Acute and/or subacute setting experience beneficial.
