Responsibilities
- Provide thorough diagnostic assessments
- Provide individual, family, and/or group therapy
- Develop treatment plans with clients and regularly review progress
- Coordinate care with patient's provider network
- Maintain accurate records of client related contacts
- Maintain patient engagement and follow up as needed
- Provide relevant referrals to patients
- Participate in 1:1 career growth meeting
