Using an evidence-based care model, youβll work closely with members, families, and caregivers to create individualized care plans that promote independence, dignity, and quality of life. This role is perfect for someone who is both compassionate and strategic, blending emotional support with practical solutions to help families thrive.

What Youβll Be Doing:
π§ Guiding Families Through Dementia Care β Develop personalized care plans that support the unique needs of each individual.
π Assessing & Advocating β Conduct comprehensive evaluations to ensure members receive the right care, at the right time.
π‘ Problem-Solving Like a Pro β Help families navigate stressful situations, tackle challenges, and adapt to changes with confidence.
π Educating & Empowering β Provide essential dementia-related education, tools, and resources to families and caregivers.
π Providing Emotional Support β Be the go-to person for reassurance, guidance, and practical solutions.
π Connecting to the Community β Link members and families to support groups, social services, and engaging activities.
π Tracking Success & Impact β Collect and manage data to evaluate care outcomes and ensure quality services.
π Sharing Inspiring Stories β Work with our Creative Director to highlight member success stories and promote community engagement.

What Youβll Need:
π Bachelorβs degree in Nursing, Social Work, Gerontology, or a related fieldΒ
π Experience in dementia care, care coordination, or patient advocacy
π» Proficiency in electronic health records (EHR) and care management software is preferred
π₯ CPR & First Aid Certified
π©Ί Negative TB Test & Background Clearance
