Overview
Oasis Child and Family Services is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to provide medical care for patients at our Recuperative Care Facility. This facility serves 5-10 patients, primarily Minnesota Health Care Program (MHCP) members experiencing homelessness, for short-term stays (up to 21 days, with a maximum of 60 days per Minnesota DHS guidelines).
The goal of the program is to support patients recovering from physical illnesses in a stable environment while transitioning them to permanent housing. The NP/PA will work closely with Case Managers, Mental Health Professionals, and support staff to ensure patient-centered care.
About the Recuperative Care Program
This program provides temporary housing and medical support for individuals who:
✔ Are unhoused and unable to recover safely in shelters or on the streets.
✔ Have been discharged from hospitals but still require short-term medical care.
✔ Need case management to transition into long-term housing solutions (e.g., group homes, assisted living).
Key Responsibilities
Medical Care Delivery
- Conduct comprehensive patient assessments, including medical history and physical exams.
- Diagnose and treat acute and chronic health conditions within the scope of practice.
- Prescribe and manage medications, therapies, and other treatments as appropriate.
- Monitor patient progress, adjust care plans, and ensure continuity of care.
Care Coordination & Compliance
- Work with interdisciplinary teams to develop and implement individualized care plans.
- Coordinate hospital discharges, referrals to specialists, and primary care follow-ups.
- Ensure compliance with Minnesota DHS regulations and Recuperative Care Program guidelines.
- Maintain accurate and up-to-date medical records in accordance with state and federal standards.
Team Collaboration & Patient Advocacy
- Work closely with Case Managers and Mental Health Professionals to address both medical and social needs.
- Provide guidance and medical expertise to support staff in delivering quality care.
- Advocate for patient needs, assisting with discharge planning and access to housing and social services.
Required Qualifications
✔ Nurse Practitioner (NP): Master’s or Doctorate in Nursing (MSN/DNP), Minnesota licensure, and national certification.
✔ Physician Assistant (PA): Graduate from an accredited PA program, Minnesota licensure, and NCCPA certification.
✔ Current DEA license with authority to prescribe medications.
✔ Experience providing care to vulnerable populations, particularly unhoused individuals or those with chronic conditions.
✔ Knowledge of Minnesota DHS guidelines and recuperative care models.
Preferred Qualifications
✔ Experience in transitional care, community health, or discharge planning.
✔ Familiarity with Minnesota housing resources and social support systems.
✔ Ability to provide trauma-informed care in culturally diverse environments.
