Responsibilities
- Deliver nursing care to hospice patients in accordance with individualized care plans.
- Assess patient needs and develop appropriate nursing interventions.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with interdisciplinary teams to ensure holistic patient care.
- Educate patients and families about disease processes, treatment options, and end-of-life issues.
- Provide emotional support and counseling to patients and their families.
- Maintain accurate documentation of patient care activities and progress.
- Participate in on-call duties as required to provide continuous patient support.
