The Advanced Practice Provider is responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients. Works closely with physicians and other clinicians in a team approach to patient care. Specialized in Women's Health / Obesity.
This is a full-time, benefits eligible position. The clinic is open Monday - Friday, 8am-5pm.
OBGYN Specialists is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
Essential Functions :
- Assess patient status by obtaining health history through patient / family interviews and chart
reviews; assess presenting illness, risk factors, and family history, psychosocial situation,
and cultural factors; and perform appropriate physical examination.
Order / perform appropriate laboratory diagnostic and other screening tests. Seek otherinformation as needed, including consultation with physicians and other clinicians, for
evaluation of illness. Integrate data to determine diagnosis and therapeutic plan, including
identification of any health risks.
Develop and implement treatment plan by prescribing / dispensing medications and / orinjections in compliance with medical practice guidelines and state laws.
Handle minor procedures.Instruct patient / family regarding medications and treatments. Educate patients regardinghealth promotion / illness prevention. Recommend appropriate community resources to meet
patient / family needs.
Communicate appropriate case management information to other professionals andcommunity agencies.
Prepare documentation for medical records including updating patient medical chart byposting examination and test results, diagnosis, medications, and treatment in
written / computerized manner.
Participate in peer reviews, chart reviews, staff education, clinical guideline development,and other continuing education and quality assurance activities to demonstrate compliance
with standards, regulations, policies, and procedures.
Act as consultant / collaborator to clinical team on area of specialty.Comply with patient confidentiality requirements.Promote patient advocacy.Collaborate with physicians in managing acute and long-term medical needs of patient.Provide monitoring and continuity of care between visits according to treatment planincluding triaging patient calls / e-mails.

Successful completion of advanced practitioner training and certification as a practitionergeneral or specialized) from an approved nurse practitioner or physician assistant program.
Current CPR certification.Graduate of an accredited Physician Assistant or Nurse Practitioner programNCCPA certificationMinnesota Board of Medical Practice registration / licenseDEA registrationExperience in family / internal medicine preferredExperience chronic diseases such a hypertension, diabetes, hyperlipidemia preferredCertificate of Advanced Education in Obesity Medicine preferredat least 3 years of primary care NP experience and certification in Obesity Management. Will accept experienced Primary Care NP who is willing to get certified in Obesity Management.Benefits & Compensation :
