Responsibilities
- Conduct thorough assessments of patients, including taking vital signs and medical histories.
- Develop and implement individualized care plans based on patient needs.
- Manage acute pain management strategies and provide IV infusion therapy as necessary.
- Collaborate with physicians and other healthcare professionals to coordinate care for patients in various settings, including ER and PICU.
- Provide infectious disease care and participate in discharge planning for patients transitioning to assisted living or home care.
- Maintain accurate medical documentation in compliance with healthcare regulations.
- Educate patients and their families on health management, treatment options, and preventive measures.
- Monitor patient progress and adjust treatment plans as needed.
Experience
- A valid Nurse Practitioner license is required.
- Proficiency in medical documentation practices is essential.
- Familiarity with discharge planning processes and patient education strategies is highly desirable.
- Previous experience managing infectious diseases or proving wound care is a plus but not required.
- Strong communication skills and the ability to work effectively within a team are crucial for success in this role.
